Abstract

Peritoneal carcinomatosis is still considered a terminal condition by many oncological groups worldwide, it is also underestimated by many others, since in many patients in whom a surgical resection is performed for curative purposes, peritoneal carcinomatosis presents as its presentation of recurrence in the following months or years.
For more than 30 years, the so-called Sugarbaker procedure has been carried out, in which, considering this condition as still surgically manageable, in these patients who have previously been extensively studied to determine the degree of extension of the carcinomatosis, a maximal cytoreduction, followed by high-flow hyperthermicintraperitoneal chemotherapy, in a procedure that takes an average of 8-12 hours. This treatment has spread throughout the world and is today considered by most countries as the standard for this condition.
This CRS + HIPEC is a very complex resective surgery that requires the presence of a multidisciplinary team from the preoperative evaluation and staging, as well as for the surgical, clinical, intensive therapy and physiotherapy support management, with hospital stays of an average of 10 days. and in which every detail counts.
It has been determined that in this type of patients in whom palliative chemotherapy is usually used, when using CRS + HIPEC, survivors can be achieved up to 10 times longer.
In Ecuador, a South American country, it has been used for 5 years, although it has not been carried out in a large number of cases, it has been possible to determine a survival greater than at least 6 times the average of the cases presented.

Fellow of the American College of Surgeons. Certification in Trauma Surgery from Hospital del Valle University Cali-Colombia. Master in obesity surgery.Certification in bariatric, hepatic, biliar and pancreatic surgery by IRCAD.Currently main surgeon at NovaclinicaModerna, Ecuadorian Social Security Institute and at Alfa Hospital.
